1. Each patient must submit their own request for a MyDovetale Activation Link.
   * You may not request a link on behalf of another individual.
2. This form will only accept requests from individuals age 16 and older.
3. If you are under 16 or wish to assist a family member, loved one or friend with access to their personal health information, email mydovetale@stjoes.ca or review our request for access protocols here.
4. MyDovetale can only be used by patients of St. Joseph’s Healthcare Hamilton.
We need some information in order to grant you a MyDovetale account. Enter your demographics here and in the next step we will verify your identity using questions from a third-party verification system. When you're approved, you will receive an email or a letter with your activation code and instructions on how to activate your MyDovetale account.
