1. Each patient must submit their own request for a MyDovetale Activation Link.
   * You may not request a link on behalf of another individual.
2. This form will only accept requests from individuals age 16 and older.
3. If you are under 16 or wish to assist a family member, loved one or friend with access to their personal health information, email mydovetale@stjoes.ca or review our request for access protocols here.
4. MyDovetale can only be used by patients of St. Joseph’s Healthcare Hamilton.
Due to the high volume of these requests and the manual processing required to activate these accounts, the activation time may be in excess of 72 hours.
Please do not resend another request or email as this may result in further delays.
- You must be 16 years of age or older
- You must have a valid Ontario Health Card Number
- You must enter an email address which belongs solely to you (not a shared account) when completing the form below
